Enrollment and Waivers

(This website will begin accepting waiver applications on July 1st.)
- Unless you make a U-SHIP coverage selection or waive U-SHIP enrollment during the enrollment period, you will be automatically enrolled in U-SHIP Basic coverage and the Office of the Bursar will bill you for the premium payments for the entire plan year (September 1 - August 31).
- The enrollment period ends at 5:00 pm on Friday of the second week of your first quarter of registration during the plan year. For Autumn Quarter 2009 the waiver submission deadline is Friday, October 9th.
- Students who wish to waive U-SHIP coverage, or would prefer Prescription Advantage coverage, must complete the enrollment/waiver process each year.
- Dependents are not automatically enrolled in U-SHIP Basic coverage. You must request that they be added to the policy each year.
- Even if you are satisfied with Basic coverage, we recommend that you confirm your enrollment as soon as possible. When the new plan year begins on September 1st, you will need a new U-SHIP ID card in order to fill prescriptions or receive medical care not covered by the Student Life Fee. Confirming your enrollment early ensures a smooth transition by allowing you to receive your U-SHIP ID card prior to September 1st.

If you will not be 18 years old by October 9, 2009, you will need to have a parent or guardian complete an Enrollment/Waiver Form for Students under 18 (pdf).
Information Required to Apply for a Waiver:
When you apply to waive U-SHIP you must provide complete policy information (name of policy holder, policy and subscriber numbers, and U.S. telephone number, as well as information on the benefits provided by your plan). You will not be able to complete a waiver application without this information.
Comparability of Coverage
Waivers will be denied if the following criteria are not met:

Additional waiver criteria:
1. Plan must have a U.S. phone number for the processing of claims information.
2. Plan must provide coverage from September 1, 2009 to August 31, 2010, or through the end of your academic program, whichever comes first.
