Dependent Coverage
- Dependent children and married spouses or registered same-sex domestic partners are eligible to be insured under U-SHIP Basic coverage for an additional charge. Dependents are not eligible for Prescription Advantage Coverage.
- Please note that dependents 14 years of age and older who enroll in the U-SHIP insurance plan will also need to pay the Student Life Fee. The benefits offered by the insurance (U-SHIP) are designed to compliment the services provided at the Student Care Center (SCC) and covered by the Student Life Fee, which means individuals need to enroll in both to recieve the full package of health care services. - To enroll a dependent spouse, domestic partner, or child in the Basic plan, you must be enrolled in either U-SHIP Basic or Prescription Advantage Coverage. Please Note: Unlike students, dependents are not automatically enrolled in U-SHIP Basic coverage. You must request that they be added to the policy each year.
- The enrollment application is completed online during the enrollment period (see Online Enrollment/Waivers on this site).
- To enroll a same-sex domestic partner in U-SHIP coverage, you must also download and complete the Statement of Domestic Partnership form. When the form is completed, return it to:
