Early Arrival Plan
For new students who need to be on campus for formal academic or intercollegiate athletic programs beginning prior to September 1, there is an early arrival plan providing Basic Plan coverage from August 18 through August 31. Students who elect this must also elect U-SHIP coverage for the 2009-10 academic year. The premiums for the early arrival plan are:
- Student only: $75
- Student + 1 Dependent (spouse, same-sex partner, or children): $208
- Student + 2 or more Dependents (spouse, same-sex partner, and 1 or more children): $323
Any deductible applied to the Basic coverage deductible during the early arrival period will be credited towards the deductible of the plan the student elects beginning September 1, 2009.
To elect the Early Arrival Plan:
- You must first enroll in the 2009-10 plan year using the 2009-2010 Insurance Enrollment/Waiver site.
- Once you are enrolled, you will be presented with a confirmation page. On that page there is a link to a pdf version of the early arrival enrollment form.
- Print and fill out this form.
- Bring, fax, or mail the completed form with payment arrangements to the on-campus StudentResources office.
If you are paying the premiums by credit card, please fax the form to StudentResources at 773-753-4544. If you are paying by check and mailing the form, please be sure to send it in a way that the mailing can be traced.
