Waiver Procedure
The application to waive the Student Accident and Sickness Insurance (SASI) plan is submitted online (see Online Enrollment/Waivers on this site) . When you apply to waive the SASI plan, you must provide complete policy information (name of policy holder, policy and subscriber numbers, and telephone number, as well as information on the benefits provided by your plan). You will not be able to complete a waiver application without this information.
If you will not be 18 years old by October 5, 2007, you will need to have a parent or guardian complete an Enrollment/Waiver Application form accessible here. The deadline for this form is October 5, 2007.
The deadline for enrollment or waiving enrollment is 5:00 pm Friday of the second week of your first quarter of registration during the plan year. If you do not apply for a waiver by the enrollment deadline, you will be enrolled in the SASI Basic plan for the entire plan year and be responsible for the premium. In addition, if you have dependents, they will not be automatically enrolled in the Basic Plan. You must request that they be added to the policy.
Every student must complete the enrollment OR waiver process each year.
