Student Accident and Sickness Insurance (SASI)

Enrollment Period

The enrollment period runs through 5:00 pm on Friday of the second week of the first quarter in which you are registered. You must make a coverage selection or apply for a waiver during the enrollment period or you will be automatically enrolled in the SASI Basic plan and you will be billed for that coverage. (See the Waiver Procedure link on this site for information on waiving coverage.)

Students approved to register as a student after the deadline must submit an insurance enrollment/waiver application at registration time. Failure to do so will result in automatic enrollment into the SASI Basic plan.

Early Arrival Plan

For new students who need to be on campus for formal academic or intercollegiate athletic programs beginning prior to September 1, there is an early arrival plan providing Basic Plan coverage from August 13 through August 31. Students who elect this must also elect SASI coverage for the 2007-08 academic year. The premiums for the early arrival plan are:

• Student only: $85
• Dependents (spouse, same-sex partner, or children): $152

Any deductible applied to the Basic Plan deductible during the early arrival period will be credited towards the deductible of the plan the student elects beginning September 1, 2007.

The enrollment deadline for the Early Arrival Plan is no later than August 7, 2007. Some programs may require an earlier deadline.

To elect the early arrival plan:

1. You must first enroll in the 2007-08 plan year at http://registrar.uchicago.edu/health/.
2. Once you are enrolled, you will be prompted to answer a question about arriving early on campus for an official academic or intercollegiate athletic program.
3. If you respond "yes," you will be presented with a PDF form.
4. Print and fill out this form.
5. Bring, fax, or mail the completed form with payment arrangements to the on-campus Chickering Office.

If you are paying the premiums by credit card, please fax the form to The Chickering Group at 773-834-4544. If you are paying by check and mailing the form, please be sure to send it in a way that the mailing can be traced.
