University Policy Regarding Health Insurance
Health insurance coverage is mandatory for all eligible students. During the time you are registered at the University, you are required to enroll in the U-SHIP Basic or Prescription Advantage coverage, or waive enrollment by providing evidence that you have health insurance coverage that is comparable. Unless you make a U-SHIP coverage selection or waive U-SHIP enrollment during the enrollment period, you will be automatically enrolled in U-SHIP Basic coverage and the Office of the Bursar will bill you for the premium payments of $2,466 for the entire 2011-12 plan year (September 1, 2011 - August 31, 2012). Every eligible student must complete the enrollment/waiver process each year to ensure uninterrupted service at the preferred level of coverage. Dependents are not automatically enrolled in U-SHIP Basic coverage. You must request that they be added to the policy each year.
Contact the on-campus representatives to enroll in or upgrade the insurance program. Please find the petition to waive at the link: http://studenthealth.uchicago.edu/studentinsurance/ Under the link Mid-Year Changes you will find a link for the Petition to Waive form. Please submit along with a front and back copy of the insurance card. The form can be faxed, emailed, or brought into the insurance office at: 5801 S. After the enrollment period, your insurance selection on file with the University will remain effective until August 31, 2012. To view your current insurance selection, login to cMore.
*Students approved to register as a student after the insurance enrollment deadline must submit an insurance enrollment/waiver application at the time of registration. Failure to do so will result in automatic enrollment into U-SHIP Basic coverage.
If you have confirmed enrollment in U-SHIP using the 2011-2012 Insurance Enrollment/Waiver site, approximately two weeks from the date of confirmation you may create an online account. From your online account you can perform tasks such as tracking your claims status.
